Neuropuncture Consent Form

Please read this document carefully and completely. This is an informed consent that explains the expectations and risks associated with Neuropuncture and Traditional Chinese Medicine. 

Nature of Treatment: Your treatment may include Neuropuncture, moxibustion, cupping, electric or magnetic stimulation, acupressure, Tui Na (Chinese medical massage), Gua Sha (dermal friction), Infra-Red (heat lamps), Chinese herbs, therapeutic exercises, and dietary counselling. 

Purpose of Treatment: The purpose of the treatment is to resolve your complaint, i.e. the reason you are seeking treatment. Neuropuncture is a health care service that is based on clinically proven prescriptions and electrical ranges for safety you can only take the prescription made by your specialist. 

Diagnosis and treatment, based on these theories are used to promote health and treat organic or functional disorders. While Neuropuncture has a great deal to offer as a health care system, it cannot replace the care of a medical physician. It is recommended that you consult a physician regarding any conditions for which you are seeking treatment(s). This is a recommendation ONLY. Benefit of Treatment: Acupuncture and Traditional Chinese Medicine procedures have been used effectively to treat a variety of diseases for hundreds of years, we combine this knowledge with the latest findings in Physiology and Anatomy, Neurosciences and research provided by Neuropuncture INC. We cannot guarantee the outcome of any course of treatment. 

Risks of Treatment: While acupuncture, Traditional Chinese medicine, and other treatments provided by this office have proven to be highly effective in correcting conditions and maintaining overall health and well-being, practitioners are required to advise patients that there may be some risks. 

Although practitioners cannot anticipate all of the possible risks and complications that may arise with each individual case, you should be aware that the following side effects can occur. If there are particular risks that apply to your case, your practitioner will discuss these with you. 

Neuropuncture is a safe method of treatment, but that it may have side effects including bruising, minor bleeding, numbness or tingling at or near the needle site, which may last a few days. Clients may also experience some muscle soreness and petechiae (due to cupping or gua sha), which may also last a few days. Drowsiness may occur in a small number of patients, and if affected, you are advised not to drive. In a small percentage of patients, nausea can occur and symptoms may become worse before they improve; this is usually a good sign. 

All prescriptions, plans and dosages are to be set by the team, practitioners and assistants of Neuromedtec. By agreeing upon the terms and conditions of this agreement the patient waivers all right to claim against any persons within the company Neuropuncture NZ LTD (trading as Neuromedtec). And that upon this agreement you acknowledge that the medicines we use include electrical stimulation, that you agree to use of needles, ASP implants, and herbal formulations- that the company its staff or directors are not liable for injury of any type.

Please advise our staff if worsening symptoms continue for more than 2 days. An unusual risk can include fainting this can be reduced with ensuring some food is consumed prior to treatment. Infection is another possible risk, however since this office uses only sterilized, disposable needles while maintaining a clean and safe environment, this is unlikely. 

Clients may stop treatment at any time. A refund may be due which will be calculated by the office based on researched hours into your case, treatment and consultations received, supplementation and administrative costs associated.  

Please note that you may wish to consult your insurance company in regards to payment of appointments, however Neuropuncture NZ do not guarantee that such a claim will be accepted by your insurance company.  Invoices that are not paid on time may incur a late fee, and invoices that are not paid at all may be passed on to a debt collection agency. 
 
By agreeing to our terms and conditions and making payment of your prescription you are giving your consent. 

Apart from your medical history, it is important to tell your practitioner if you have ever had a fit, seizure, have fainted, or have experienced any odd sensations. Please let us know if you have a pacemaker or electrical implants. Let us know if you have a bleeding disorder, are taking anti-coagulants, any other medication, have a risk of infection, or have a damaged heart valve. Please notify the practitioner, who is caring for you, if you become pregnant.

Statement of Consent 

"I confirm I have read and understood the above information, and I consent to having treatments and procedures from Neuropuncture NZ (Trading as Neuromedtec). I have read the possible risks of treatment outlined above, but do not expect the practitioner to be able to anticipate and explain all of the possible risks and complications of treatment. I wish to rely on the Practitioner to exercise judgments during the course of treatment and decide what they think is in my best interest, based upon the facts that are known at the time. I understand the practitioner and administrative staff may review my medical records, but all of my records will be kept confidential and will not be released without my written consent. By voluntarily agreeing below I show that I have read this consent form, have been told about the risks and benefits of treatments provided by this office and have had an opportunity to ask questions. I intend this consent form to cover the entire course of treatment for my present condition and further conditions for which I seek treatment."

Neuromedtec Online Education

Terms and Conditions

1.  This Agreement is between you and Neuromedtec LTD and relates to your participation in our online therapy.

2.  All references to “Neuromedtec”, “We”, or “Us” or “Our” are references to Neuromedtec LTD.

3.  Users of Neuromedtec Online Therapy are referred to below as “members”, “participants”, or “you” or “I” as the context requires.

4.  The following sets out the terms and conditions for use of the Neuromedtec website, Facebook page, YouTube channel, Zoom, GoToWebinar and the use of our online live-streamed or pre-recorded programs.

5.  We reserve the right to vary this agreement and revise the content of the Digital Course, reschedule any bookings or to make other changes to the course content and program as reasonably required.

6. By participating in Neuromedtec Online Therapy or in discussions between us and you, or generally among Neuromedtec members, you expressly acknowledge and accept that:

a)  Neuromedtec does not provide any warranties or guarantees as to any specific result from participation in the Medical Movement Therapy lessons.

b)  While some general guidence or discussion may from time to time be provided within Neuromedtec Online Therapy on topics touching on professional health service areas, you accept that Neuromedtec and other members are not purporting to replace such services. Neuromedtec does not purport to endorse the comments or advice of other members or other participants, or its accuracy or completeness. Professional advice should be sought where necessary, in particular if you have physical disabilities or health conditions.

c)  The Medical Movement Therapy provided by Neuromedtec follow many different practices. You are solely responsible for ensuring you have a sufficient degree of fitness and physical capability to participate in these Medical Movement Therapy formats, and/or to seek professional advice from a medical professional or contact Neuromedtec if you are unsure.

d)  If experiencing any unusual pain, discomfort, headaches, dizziness, nausea, or emotional instability during any Neuromedtec Medical Movement Therapy lessons, the participant should cease the practice and seek advice from a medical professional or contact Neuromedtec before continuing the lessons.

e)  Neuromedtec will not be legally responsible for or bear any legal liability for any harm or injury suffered by any participant as a result of their participation in Neuromedtec Online Therapy and shall not accept any claim for compensation or reimbursement of medical costs associated with this.

f)  To the extent any other legal claim is otherwise permitted by law and is established against us, our total liability in respect of all claims in connection with any services provided by Neuromedtec shall be limited to the total sum of any membership fees paid by you that relate to the cause of action established. You shall be bound by and observe the conditions and restrictions of use or sharing of Neuromedtec content and associated intellectual property as set out in Clauses 7-12.

g)  During the sessions, you may be included in the sharing of confidential information. Please choose to respect the participant/speaker and refrain from discussing this outside the sessions.

h)  Neuromedtec LTD programmes are a full course commitment. There are no cancellations or refunds during this time. If you use in house finance and should your credit card expire, you will be required to supply new details that will take you up to the end of the agreed payment period. We reserve the right to suspend your access until your payments are up to date.

i)  The clients/patients of Neuromedtec LTD understand that they will conduct themselves ethically throughout the programme. If conduct is deemed unacceptable and not corrected after notice, Neuromedtec LTD reserves the right to cancel your enrolment.

CONDITIONS OF, AND RESTRICTIONS AGAINST, USE OR SHARING OF NEUROMEDTEC AND ASSOCIATED INTELLECTUAL PROPERTY

7.  The name “Neuromedtec”, The Neuromedtec Logo, and all content produced by Neuromedtec are the intellectual property of Neuromedtec and may not be used in any manner that constitutes a breach of our intellectual property rights, unless expressly permitted by Neuromedtec.

8.  All Neuromedtec content is for your personal use only. Personal use does not include broadcasting or sharing the content among large groups or public audiences, or in any online group or forum whether public or private, unless you have the express prior written consent of Neuromedtec.

9.  You must not use, share, post or re-post Neuromedtec material in connection with, or to promote, other services or products in any other forum, whether online or otherwise, without the express written permission of Neuromedtec.

10.  As a participant of Neuromedtec LTD you understand that all information, techniques, skills and concepts that are shared by physicians and practitioners and all online video and audio recordings, all audio and video trainings, manuals, information, checklists, documents, websites and other materials are the property of Neuromedtec and are for education only.

11.  Neuromedtec Programs and the associated Materials may only be used for educational purposes and only for non-commercial use.

12.  It is assumed that this application is completed with the understanding that what is taught within Neuromedtec LTD remains the copyright of Neuromedtec LTD and will not be replicated, modified, licensed, published, transmitted, distributed, uploaded, broadcast, sold or otherwise transferred without our consent.

PRIVACY AND MARKETING

13.  By participating in Neuromedtec Online Therapy you accept that:  

a)  Any comments or posts you make may be visible to all members of Neuromedtec. Because of the way in which Facebook posts and Zoom discussions are published, you also accept that your comments may be visible to other Facebook and Zoom users outside of the Neuromedtec group.

b)  Neuromedtec may re-post Neuromedtec Online Therapy lessons to forums online in order to promote and encourage others to join the group, and that this shall not constitute a breach of your privacy by Neuromedtec.

c)  Neuromedtec will use all reasonable endeavours outside of the circumstances in 13.a and 13.b to respect your individual privacy. Should we wish to use any specific information about you, including posts about your personal experience in Neuromedtec or photos you publish in Neuromedtec as part of any wider online promotional or marketing activities, we will seek your permission.

d)  Members and third parties are not permitted to advertise or promote their products or services on Neuromedtec forums without the express written consent of Neuromedtec.

e)  If you agree to be interviewed for marketing and training purposes you give permission for the video to be used for marketing and training purposes. 

  • These terms and conditions are governed by the laws of New Zealand.

 

  • By clicking “I agree” or accessing and participating in our Online Therapy, you agree to be personally bound by these Terms and Conditions, including any changes or modifications made to these Terms and Conditions from time to time, according to the laws of New Zealand, and you warrant that you are at least 18 years of age or have the permission of a parent or caregiver to participate in Neuromedtec Online Therapy.
